Objective of the Clinic

To provide primary healthcare and health education to all eligible Kosciusko County Residents

To provide primary health care services to all eligible residents of the service area regardless of race, creed, color, sex or any other characteristic as protected by Indiana Law

To actively promote programs of service and community education that seek to prevent chronic illness of any kind

Services Provided

Basic primary health care

Treatment and management of chronic and acute health conditions

Provide referrals to specialists as needed

Education regarding chronic/New health challenges

Referrals as needed for additional testing
Patient Responsibilities
There is NO CHARGE for care given at the clinic.

St. Anthony’s is NOT responsible for charges from providers as a result of referrals to other providers or for appointments not kept with other providers.

If a referral appointment is made for a clinic patient and the patient cannot keep it, the patient is responsible for cancelling or rescheduling.

Transportation is not provided to the clinic or referral appointment locations.
